Minnesota Department of Health Diabetes Program

The I CAN Prevent Diabetes Bringing the National Diabetes Prevention Program to Minnesotans is part of the collaborative work in Minnesota to increase consumer awareness of prediabetes and that type 2 diabetes can be delayed or prevented by following the National Diabetes Prevention Program. We are also engaging communities to promote this message. In order to increase consumer awareness and participation in the National Diabetes Prevention Programs in Minnesota, we are also providing information for providers to help them identify patients with prediabetes, inform them of their risk of developing type 2 diabetes, and refer patients with prediabetes to a National Diabetes Prevention Program (NDPP) near them. Our goal is to have NDPP group programs available across Minnesota.

This work can only be completed in partnership with many organizations in Minnesota. We acknowledge the many sites and organizations that already offer the National Diabetes Prevention Program under many different names. We have compiled the lists of all these programs to assist you in finding a group near you.

The Minnesota Department of Health is also working in partnership with local public health and the Statewide Health Improvement Program (SHIP) to promote these programs and to find ways to offer more programs. If you are interested, contact your local public health SHIP Coordinator.
